Knowledge Management ('KM') comprises a range of practices used by organisations to identify, create, represent, and distribute knowledge. KM is more about transforming of data into information. The information is stored and made it available for others for future usage.

This looks like a very nice, concise definition of KM. I'm worried I can't see anything wrong with it. ;-)

This is a good definition as Mark mentioned but probably you reached this definition by reading many others. In all cases it would be more powerful & convencing if you support some of the words you want to emphasise with references.
